Points to note when filling out the application
Before filling out the exam registration form, candidates must carefully read the items and detailed instructions on the back of form number 2. If there is anything unclear, candidates must ask the officer collecting the application for full instructions.
Candidates must fill in all the items on the front of the application bag, form number 1 and form number 2, then submit them to the receiving place with a photocopy of both sides of the ID card on one side of an A4 sheet of paper and 2 new 4x6 ID photos taken within the last 6 months (with full name, date of birth, province, district and registration unit code clearly written on the back of the photo, these 2 photos are contained in a small envelope). In addition, 1 more photo must be pasted in the designated position on the front of the application bag. The high school where the candidate is studying or the local police station where the candidate is residing must sign and stamp the photo (only to confirm the candidate's identity). Candidates are fully responsible for the information declared in the application form.
The place where the documents are collected will keep the document bag, form No. 1, a photocopy of the ID card and 2 photos, and return form No. 2 to the candidate after signing and stamping the confirmation. The candidate will keep this form No. 2 to receive the exam notice and certificate of exam results. In case the exam notice is lost or there are errors before the exam, the candidate will bring this form directly to the Examination Council at the registered Examination Cluster at the meeting to disseminate the regulations to complete the exam procedures.
Candidates must keep their accounts confidential.
The application file includes 02 application forms, a photocopy of both sides of the ID card on 1 side of A4 paper, 02 4x6 photos and an envelope with the candidate's full name and address in the application bag according to the form of the Ministry of Education and Training.
After submitting the Registration Form, the candidate will be given an account and password by the Registration Unit to log into the QLT software via the internet at http://thisinh.thithptquocgia.edu.vn . This account is used by the candidate to log into the QLT software from the time of registration until viewing the exam results, considering recognition of high school graduation, considering university and college admission . To ensure information security, when receiving the account and password, the candidate should change the password immediately. Candidates need to keep their account and password confidential.
In case candidates forget their account and password, they can request a new one by contacting the Registration Unit, or using the function to send the password to the email (email address stated in the Registration Form) on the system.
Depending on the time, when logging into the QLT software, candidates can know information such as: Registration information (feedback on errors if any before May 5, 2015); High school graduation recognition information (feedback on errors if any before June 5, 2015); Exam notice; Exam location; High school graduation recognition results; University and college admission results.

According to regulations of the Ministry of Education and Training, high schools are responsible for guiding candidates to fill in the Exam Registration Form completely and correctly, especially information about preferential treatment in university and college admissions .
Specifically, review the application for recognition of high school graduation to ensure the accuracy of the information filled in by candidates in the Application for recognition of high school graduation, especially the priority regime to receive bonus points and priority points; take active measures to help candidates have enough certificates to receive bonus points and priority points (if any); consider and decide on the candidate's exam conditions. Note: absolutely do not accept invalid applications.
Candidates register for the exam at the examination cluster according to the regulations of the Ministry of Education and Training. Information on province code, district code, commune code, high school code, region, and priority subjects is regulated by the Ministry of Education and Training. Candidates taking the exam for the purpose of only being considered for recognition of high school graduation or only being considered for admission to university, college , or both purposes can register for the exam at the examination cluster chaired by the university. The examination cluster chaired by the Department of Education and Training organizes the exam for candidates taking the exam for recognition of high school graduation only.
For Foreign Language, candidates can choose to register for one of the following languages: English, Russian, French, Chinese, German or Japanese. Candidates can register for a Foreign Language other than the Foreign Language they are studying in high school.
Candidates can use a foreign language certificate other than the foreign language they are studying at high school to be exempted from the foreign language exam in the 2015 high school graduation recognition consideration.
Instructions on how to claim priority and incentive points are as follows:
I. GRADUATION PRIORITY AREA Candidates belong to one of the following categories:
1. Area 1 : No priority points are awarded .

2. Area 2 : add 0.25 points for candidates belonging to one of the following categories:
a) - War invalids, sick soldiers, people who enjoy policies like war invalids, sick soldiers with reduced working capacity of less than 81% (only for GDTX); Children of war invalids, sick soldiers, people who enjoy policies like war invalids, sick soldiers with reduced working capacity of less than 81%; Heroes of the People's Armed Forces, Heroes of Labor; Symbol : D2-TB2
- Children of Armed Forces Heroes, Labor Heroes, Vietnamese Heroic Mothers. Symbol : D2-CAH
- Ethnic minorities; Symbol D2-TS2
- Kinh people, foreigners residing in Vietnam with permanent residence registration for 3 years or more (as of the exam date) in extremely disadvantaged communes, border communes, safe zone communes under the investment program 135; in extremely disadvantaged communes in coastal and island areas according to current regulations of the Prime Minister; in extremely disadvantaged villages, communes in regions I, II, III in ethnic and mountainous areas according to current regulations of the Ethnic Committee, studying at general schools not located in inner-city districts of centrally-run cities for at least 2/3 of the time studying at high school level; Symbol : D2-VS2
- People infected with Agent Orange; children of people infected with Agent Orange; children of resistance fighters infected with toxic chemicals; people recognized by competent authorities as having deformities, disabilities, or reduced ability to self-reliant in daily life or work due to the consequences of toxic chemicals; Symbol : D2-CHH
- Children of revolutionary activists before the August 19, 1945 General Uprising (for GDTX); Symbol : D2-CCM
- Age 35 years or older, as of the exam date. Symbol : D2-T35
3. Area 3: add 0.5 points for candidates belonging to one of the following categories:
- Ethnic minorities, who have permanent residence in extremely disadvantaged communes, border communes, and safe zone communes under the investment program 135; in extremely disadvantaged communes in coastal and island areas according to current regulations of the Prime Minister; in extremely disadvantaged villages, communes in regions I, II, III in ethnic and mountainous areas according to current regulations of the Ethnic Committee, studying at ethnic boarding schools or general schools not located in inner-city districts of centrally-run cities; Symbol: D3 - TS3
- War invalids, sick soldiers, people enjoying policies like war invalids and sick soldiers with reduced working capacity of 81% or more (for GDTX); Symbol : D3-TB3
- Children of martyrs; children of war invalids and sick soldiers; people who enjoy the same policies as war invalids and sick soldiers and have a reduced working capacity of 81% or more. Symbol : D3-CLS
* Students with multiple priority criteria will only benefit from the highest criteria.
II. RECOMMENDED POINTS:
1. Won individual prize in the 12th grade cultural subject excellent student contest:
- First, second, third prize at national level or first prize at provincial level: 2.0 points.
- National encouragement prize or provincial second prize: 1.5 points.
- Third prize at provincial level: 1.0 point.
2. Winning individual and team prizes in practical experiments in Physics, Chemistry, Biology; art competitions; physical education and sports; national defense education sports competitions; science and technology competitions; international letter writing competitions organized by the Education sector in coordination with specialized sectors from provincial level and above at high school level:
- Winning first, second, third national prizes or first provincial prizes or Gold Medal: 2.0 points.
- National encouragement prize or provincial second prize or Silver Medal: 1.5 points.
- Third prize at provincial level or Bronze Medal: 1.0 point.
* Team prizes are only counted for national prizes, the additional points are the same as individual prizes.
* Students who win multiple awards will only receive the bonus points of the highest award.
3. Certified profession:
- Excellent: 2.0 points.
- Fair: 1.5 points.
- Average: 1.0 point.
4. If GDTX students have a Foreign Language A or IT A certificate or higher (including technicians): 1.0 point will be added for each type of certificate.
* Maximum incentive points for items 1,2,3,4 above are not more than 4.0 points.
* The incentive points specified in items 1,2,3,4 above are reserved throughout the entire grade level./.
